Electronic Change of Grade Form

When instructors fill out the form, they will be asked to provide the name and email of their unit’s grade coordinator. The Math Department’s grade coordinator is Margaret Somers, msomers@ufl.edu.

Some tips from the registrar’s office:

  • If you do not know your class information (course number, class number), please search for it using the Schedule of Courses (SOC)
    • Subsequently, if you are unsure on who is approving your grade change, search for your class in the SOC and the department will be listed.
  • Do not enter a space in between the course number (LAT1120 instead of LAT 1120).
  • This form is only for Change of Grade (COG) requests for courses in Fall 2018 – onwards. Previous data will not be available, please use the secure upload for older grade changes, submit through your college and we will process manually.
    • IF the grading calendar for a specific course is still open, the form will not allow you to submit a change of grade.
  • Please use the latest versions of Firefox and Chrome.
  • If you are submitting many COG requests, enter the class information, click Continue, then save the following URL. The URL will save the class information so you do not have to enter it every time.
  • Please note, some credits may appear as “0.” This has been reported to IT, but will NOT negatively affect the COG in any way.
