Electronic change of grade form

When instructors complete the form, they will be asked to provide the name and email address of their unit’s grade coordinator. Margaret Somers is the Department of Mathematics grade coordinator: msomers@ufl.edu.

Tips from the registrar’s office

  • If you do not know your class information, such as the course number or class number, search for it in the Schedule of Courses (SOC).
    • If you are unsure who approves your grade change, search for your class in the SOC; the department will be listed.
  • Do not enter a space in the course number (enter LAT1120 instead of LAT 1120).
  • This form is only for Change of Grade (COG) requests for courses from Fall 2018 onward. Data from earlier terms are unavailable. For older grade changes, use the secure upload and submit through your college for manual processing.
    • If the grading calendar for a specific course is still open, the form will not allow you to submit a change of grade.
  • Please use the latest versions of Firefox and Chrome.
  • If you are submitting many COG requests, enter the class information, click Continue, and then save the following URL. The URL will retain the class information so you do not have to enter it every time.
  • Some credits may appear as “0.” This issue has been reported to IT, but it will not negatively affect the COG request.
